Welcome! My name is Paige Austin, and I am the school nurse here at Groves Elementary. I look forward to an exciting year and to caring for the health and well-being of all the students of GE. If at any time you have any questions or concerns, please feel free to contact me at (409) 962-1531 x6104.
Keep the Nurse Informed
- Food allergies and medical information are obtained from parents/guardians during the completion of online registration; however, should your child have any health conditions or require special accommodations due to a health related issue, please contact me to discuss.
- Please call the office in the morning if your child is absent for any reason. If the absence is due to illness, please inform us of the symptoms (fever, cough, sore throat, etc.) as I track illnesses for infection control purposes.
- Please inform me if your child will be out for an extended period of time due to illness, chronic medical condition, surgery, and/or hospitalization. If any special accommodations will be required upon your child's return, please let us know, so that we may make arrangements to promote your child's health and academic success.
Medication Information
- Never send any type of medication to school by your child! Medication must be brought and checked in with the nurse by the parent/guardian. No exceptions!
- Should your child require medication (prescription or over-the-counter) while at school, all proper forms must be in place in order for me to administer the medication. All medications must be in their original containers. No Ziploc bags or unlabeled bottles will be accepted. Prescription medication must be in a dated, labeled, pharmacy container, and this includes inhalers and Epi-Pens.
- All prescription medications require a "Physician's Request for Administration of Medication" form, which must be completed and signed by the healthcare provider and signed by the parent/guardian. Antibiotics ordered 3 times a day or less should be given at home.
- All over-the-counter medications require a "Parent Request for Medication to be Administered" form, which must be signed by the parent/guardian.
- If your child requires to carry an asthma inhaler with him/her at all times, a "Physician's Request for Self Administration of Asthma Inhalers" form must be completed and signed by the healthcare provider and signed by the parent/guardian.
- Forms may be obtained from the nurse's office or this website.
- Any changes in medication will require a new form.
- A new medication form will be required at the start of each school year as forms are only valid for one school year.
- Any unused medication will only be returned to the parent/guardian. No medication will be stored over the summer. Any medications left at the end of the school year will be discarded.
